U.S. Passport Application Forms (DS-11, DS-82, DS-5525)
Download the current federal passport application forms. These are not notarized — DS-11 is signed in person before an acceptance agent — but we host them so your paperwork is in one place.
What it is
The core U.S. passport forms: DS-11 (first-time / in-person application), DS-82 (eligible mail-in renewal), and DS-5525 (statement of exigent or special family circumstances).
Who needs it
Anyone applying for or renewing a U.S. passport. Pair the DS-3053 with a DS-11 when one parent can't appear for a child's application.

How it works
- 1Pick the right form: DS-11 for first-time or in-person, DS-82 for eligible renewals by mail.
- 2Do not sign a DS-11 in advance — you sign it in front of a passport acceptance agent.
- 3Bring supporting documents and photos per the form instructions.
Common questions
Do these need to be notarized?
No. The DS-11 is signed before a passport acceptance agent, and the DS-82 is a self-signed mail-in renewal. Only the DS-3053 consent form is notarized.
